The heart failure specialists on the medical staff use innovative and advanced treatment approaches designed to manage heart failure symptoms and help guests learn to manage their heart failure condition. For guests requiring heart failure treatment, the physicians on the medical staff of the Heart Failure Center will coordinate with physicians on the medical staff at The Heart Hospital and can refer to surgical services to provide individualized care to address your unique concern. Treatment approaches may include:

For patients with advanced heart failure, the Heart Failure Center offers innovative destination treatment, including a Ventricular Assist Device Program certified by The Joint Commission. For guests needing a heart transplant​, physicians affiliated with Outpatient Services can refer to a Baylor heart transplant facility.

Heart Failure Treatment at Outpatient Services

Although diagnosed and initially treated at The Heart Hospital, much of the follow-up, daily management, and ongoing treatments of heart failure are conducted by physicians on the medical staff at the Heart Failure Center at Outpatient Services​. Outpatient Services is an outpatient facility owned and operated by Baylor Scott & White The Heart Hospital – Plano.

The goal of the Heart Failure Center is to help you manage the day-to-day challenges of heart failure. Through patient-focused educational initiatives, the board-certified advanced heart failure physicians on the medical staff, along with nurses and nurse practitioners with specializations in heart failure, and the rest of the team at the Heart Failure Center help you understand the day-to-day experience of living with heart failure. The goal is to help you learn about heart failure, understand your medications and the importance of regular follow-up appointments. The heart failure specialists also work closely with you to provide a smooth transition home if you are hospitalized.

The heart failure specialists on the medical staff are in regular contact with your personal physician. Although your heart failure care may be provided at Outpatient Services, your regular cardiologist and/or primary care physician are kept in the loop. If needed, the heart failure specialists can also coordinate care with a wide range of specialists, such as electrophysiologist​​​​​s or surgeons.

Heart Failure Support Services

The Heart Failure Center provides support services and regular consultations to assist you with daily living activities as they relate to your heart failure, including:

  • One-on-one consultation to address your specific needs and concerns
  • Dietary consultation with a registered dietitian to address appropriate nutritional concerns and weight management
  • Social services to help coordinate care and navigate insurance issues
  • At-home care guidance
  • Pharmacist availability for medication management​​​​
