The "Heart Screenings Registration Form" can be completed and submitted if you have scheduled a "Cardiac Calcium Score Heart Screening" or a "Living for Zachary Screening." It cannot be used for any other type of hospital procedure. Please note: online pre-registration must be submitted 72 hours prior to your scheduled appointment.

The "Heart Procedures Registration Form" can be completed and submitted for any heart procedure you have been scheduled to have at THE HEART HOSPITAL Baylor Plano. It should NOT be used/submitted if you are coming to the hospital for a "Cardiac Calcium Score Heart Screening" or a "Living for Zachary Screening." Please note: online pre-registration must be submitted 72 hours prior to your scheduled appointment.
